Public Service Announcement Submission Form

The City of Hopkinsville website will post public service announcements from recognized governmental agencies, civic or religious organizations, churches, and public and private schools. The announcement must be submitted at least two weeks in advance of the event. Public Service Announcements can not be used to advertise commercial products or services.

To submit a Public Service Announcement, please complete the following form:

Name of Organization/Group (Required)

Your E-Mail Address (Required)

Your Phone Number (Required)

Upload PSA file (Preferred Method of PSA Submission)

This form can be used to upload Images, PDFs, DOCs, PowerPoints, or Publisher files.

PSA Distribution Options (Required)

Please indicate whether you would like your PSA to run on City TV, be displayed on the City of Hopkinsville website, or both.

Please make sure to include the desired posting dates for the PSA. If you uploaded a PSA file, it is not necessary to include information about your PSA that is included in that file. Otherwise, please include all relevant information about your PSA in the following field.
